Commissioning tested the components, not the failure.

What happened

The Harbour Solutions programme delivered three wastewater treatment plants at a total of approximately $333 million. In January 2009, an incident at one of the plants caused damage of approximately $54 million.

The plant was offline for approximately 14 months. During the outage, untreated sewage was discharged into Halifax Harbour — the outcome the programme had been built to end.

The information failure

Technology and Process, at the boundary where a built asset becomes an operated one. Commissioning had verified that the components worked. It had not tested the integrated failure scenarios — what happens when several conditions occur together, and what the plant does when they do.

That distinction is an information requirement. Whether commissioning covers component performance or system failure behaviour is something that is specified, or is not, in the documents that govern handover.

What would have been different

Integrated failure-scenario testing, specified as an information requirement at procurement and gated at handover, is the control. The specification without the gate is a request; the gate without the specification is unenforceable. Both are needed, and both are ordinary documents.

Halifax Water has published extensively on the incident and its response. We cite it here because it is documented, not because the organization was unusual.
